Send a referral

Address referrals to Dr Joshua Hurwitz, Medical Oncologist, Oncology Clinics Victoria, and send them by fax, email or HealthLink.

Let us know the patient's preferred location:

What to include

• Patient details, including Medicare number and a contact phone number
• Reason for referral and how urgent it is
• Relevant history, current medicines and allergies
• Histopathology, including receptor or biomarker results where available
• Recent imaging reports, and where the images are held
• Preferred location, or whether telehealth is needed
• Any interpreter or accessibility needs
